Position Overview

The Case Manager at Music City Detox is responsible for the strategic planning, execution, and management of personalized client services throughout their stay and in preparation for ongoing care and secure, sober living arrangements post-discharge.

Key Responsibilities
  • Conduct patient evaluations, intake procedures, and initial assessments.
  • Create or adjust a tailored service plan based on assessments and relevant data; facilitate referrals for identified services; arrange transportation for clients; and ensure follow-up on referrals during residency and for a minimum of 30 days after discharge.
  • Provide individual case management sessions in alignment with care level requirements and document findings in the client's records.
  • Support clients in fostering motivation for recovery and participation in recovery-related activities.
  • Assist clients in identifying and connecting with essential resources for immediate needs and future levels of care, including safe and sober housing and additional recovery support.
  • Lead psychoeducational and motivational enhancement group sessions.
  • Participate in staff meetings, case discussions, and necessary training to collaborate with the program team and uphold quality care standards in accordance with clinical policies.
  • Maintain collaborative relationships with agencies, community resources, and families to coordinate patient care services; provide information on substance abuse treatment options; make necessary referrals; and ensure follow-up to confirm the delivery of appropriate services.
  • Perform additional duties as assigned.
Qualifications

Education:

  • A Bachelor's Degree from an accredited institution, or an equivalent combination of education and experience, is required.

Experience:

  • Proven knowledge and experience in case management is essential.
  • Experience in substance abuse, mental health, healthcare, housing, and community support services is required.
  • Experience in building rapport with individuals facing substance abuse challenges and navigating referral and support systems is highly preferred.
  • Experience in managing patient services and maintaining both computerized and manual records is strongly preferred.
  • Familiarity with compliance regarding internal, local, state, and federal regulations is strongly preferred.
