Job Overview

The Client Services Coordinator is responsible for planning, executing, and managing a comprehensive range of personalized client services throughout their stay and in preparation for ongoing care and secure, sober living after discharge.

Key Responsibilities:

  • Conduct patient screening, intake, orientation, and assessment.
  • Create or adjust a personalized service plan based on assessments and other relevant data; make necessary referrals for services identified in the plans; arrange for patient transportation; and follow up on referrals during residency and for a minimum of 30 days post-discharge.
  • Facilitate individual case management sessions according to care level requirements and document the progress in the client's records.
  • Support patients in cultivating motivation for recovery and participating in recovery activities.
  • Assist patients in identifying and connecting with suitable resources for immediate needs and future levels of continuing care, safe and sober housing, and additional recovery support.
  • Lead psychoeducational and motivational enhancement group sessions.
  • Participate in staff meetings, case conferences, and required training to collaborate with the program team and ensure quality care and continuity in line with clinical policies.
  • Maintain effective relationships with agencies, community resources, and families to coordinate patient care services; provide information on substance abuse treatment resources and services; make necessary referrals; and facilitate follow-up to ensure appropriate service delivery.
  • Perform other duties as assigned.

Essential Skills and Qualifications:

  • Detail-oriented with strong organizational skills.
  • Able to thrive under pressure and meet deadlines while being flexible and reliable.
  • Excellent interpersonal and analytical abilities.
  • Outstanding customer/client service skills with a focus on resolving service issues.
  • Comprehensive knowledge of relevant standards and regulations.
  • Ability to utilize logic and reasoning to evaluate the strengths and weaknesses of various solutions, conclusions, or approaches to challenges.
  • Capacity to manage multiple priorities with a sense of urgency.

Educational Requirements:

  • A Bachelor's Degree from an accredited institution, or an equivalent combination of education and experience, is required.

Experience Requirements:

  • Proven knowledge and experience in case management are essential.
  • Experience in substance abuse, mental health, health, housing, and community support services is required.
  • Experience in building rapport with substance-abusing populations and navigating referral and support systems is strongly preferred.
  • Experience in managing patient services and maintaining computerized and manual records is strongly preferred.
  • Experience in adhering to internal, local, state, and federal regulations is strongly preferred.
