Overview
As a Patient Care Coordinator at Covenant Health, you will play a vital role in supporting women with high-risk pregnancies. Our team is dedicated to guiding patients through their healthcare journey from conception to delivery, ensuring they receive the specialized care they need.

With over 25 years of experience, we have successfully managed high-risk pregnancies and delivered thousands of healthy babies. Our services cater to various high-risk scenarios, including advanced maternal age, multiple gestations, and medical complications.

Our main office is committed to providing comprehensive care, with additional locations to ensure accessibility for all patients.


Position Summary:

In this role, you will be responsible for orchestrating patient care across various healthcare settings. Your primary focus will be on achieving optimal clinical outcomes while managing costs and resources effectively. This will involve a collaborative approach that includes:

  • Assessing patient needs through consultations with healthcare providers and reviewing medical histories.
  • Recommending appropriate care placements based on clinical evaluations and available resources.
  • Identifying opportunities for intervention to enhance patient care and education.

Additionally, you will:

  • Facilitate patient appointments and ensure continuity of care among all healthcare providers.
  • Connect patients with community resources to promote health and well-being.
  • Act as the primary contact for patients, addressing inquiries and providing support.
  • Participate in interdisciplinary team meetings to discuss patient needs and discharge planning.
  • Analyze barriers to care and collaborate on solutions to improve patient outcomes.

Qualifications
Minimum Education:
Education requirements are determined by the necessary licensure and certification standards.

Minimum Experience:
A minimum of three years of clinical experience in a women's services environment is required.

Licensure Requirement:
Must hold a current, valid license in good standing with the relevant state authority.
