Key Responsibilities
  • Coordinate patient care services to ensure a smooth continuum of care.
  • Act as a liaison between patients, families, treatment teams, and external providers.

  • Advocate for patients and families, identifying and connecting them with necessary resources.
  • Perform comprehensive psychosocial assessments and provide ongoing patient assessments.
  • Ensure patients receive care at the most appropriate level.
  • Promote efficient, coordinated care with multidisciplinary treatment teams.
  • Facilitate patient-level care through coordination and implementation of services.
  • Utilize community resources and support services, providing referrals as needed.
  • Implement cost management strategies to reduce readmissions and ED utilization.
  • Verify patient and family education and training.
  • Stay up-to-date on regulatory standards and quality measures for participating payers.
  • Follow policies, procedures, and safety standards.
Requirements
  • Masters' degree from a CSWE-accredited graduate social work program.
  • Two years of experience in a clinical social work setting.
  • Licensed Clinical Social Worker (LCSW) required.
