About the Role

As a Patient Care Coordinator at CenterWell Home Health, you will play a vital role in delivering high-quality, patient-centered care in the comfort of their own homes. Our team is dedicated to providing comprehensive home health services that cater to the unique needs of each patient.

Key Responsibilities
  • Develop and Maintain Relationships

Establish and maintain strong relationships with key hospital, skilled nursing, and assisted living facility discharge planning services to ensure seamless transitions of care.

Conduct Facility Visits

Visit facilities at the request of physicians to assist the program clinical team in determining patient eligibility for our services.

Ensure Effective Communication

Collaborate with program staff and other field sales resources through regular meetings to ensure effective communication and coordination of care.

Process Physician Orders

Assist in the timely processing of physician orders as directed by the program clinical team.

Requirements
  • Education and Experience

A RN/LPN/LVN license is required. A Bachelor's of Science in Nursing is preferred, as is a minimum of three years of clinical experience.

Skills and Qualifications

Excellent customer service, account development, organizational, time management, problem-solving, communication, and selling skills are essential for success in this role.
