POSITION OVERVIEW - CLIENT SUPPORT SPECIALIST

The Client Support Specialist is tasked with managing a designated caseload of individuals with dual diagnoses who are involved with the justice system and require mental health competency restoration services. The primary duties include delivering direct care services aimed at restoring competency through the assessment of treatment needs, development, monitoring, evaluation of treatment plans and progress, and facilitating interdisciplinary collaboration. This role necessitates sound judgment and adaptability in crisis situations, ensuring follow-up with supportive services for participants, and providing focused oversight, communication, and coordination. This is a 24/7 program and may necessitate night and weekend shifts to ensure the safety and well-being of all clients in the facility. Shifts may be adjusted to align with the operational needs of the program. Occasional travel may be required for meetings with contracting agencies.

  • Conducts assessments of daily living skills (ADLs), coordination, monitoring, planning, referral, and follow-up with comprehensive community services and supports.
  • Assists clients in acquiring resources and navigating applications for benefits and entitlements in collaboration with the Benefits/Navigation Team.
  • Coordinates and oversees the delivery of services to clients, including medical, social, educational, and vocational support.
  • Helps clients secure essential needs such as housing, food, clothing, medical and dental care, legal, vocational, and psychiatric services.
  • Connects clients to both community-based and internal resources, including transportation services, peer support, work programs, and navigation assistance.
  • Practices trauma-informed care.
  • Collaborates with other service providers and community resources to ensure clients receive comprehensive support.
  • Participates in case conferences and team meetings to ensure effective integrated service delivery.
  • Maintains accurate, timely, and up-to-date client records and documentation.
  • Utilizes Evidence-Based Practices (e.g., IDDT, DBT, CBT, psychoeducation, Motivational Interviewing, etc.).
  • Provides crisis intervention and support to clients as necessary.
  • Performs additional duties as assigned.
EDUCATION AND EXPERIENCE:

A Bachelor's degree from an accredited institution in psychology, sociology, social work, counseling, or a related human services field is required.
