Job Summary

We are seeking a highly skilled and compassionate Patient Care Coordinator to join our team. As a Patient Care Coordinator, you will play a vital role in facilitating the planning and organization of patient care activities, working closely with our healthcare professionals to ensure seamless care delivery.

Key Responsibilities

  • Coordinate patient care activities, including non-clinical screenings and referrals
  • Identify eligible resources for patients and navigate resources to ensure timely access to care
  • Assist with the referral process and coordinate provider appointments
  • Communicate directly with patients and families to ensure timely and effective care delivery
  • Maintain accurate and complete documentation of patient records
  • Ensure confidentiality and patient privacy procedures are upheld at all times

Requirements

  • High school diploma or equivalent required
  • At least two years of experience in a professional office environment
  • Excellent verbal communication skills and ability to work with confidential information
  • Proficiency with Microsoft Office and other software applications
  • Commitment to service excellence and patient satisfaction

Preferred Qualifications

  • Experience coordinating care for patients, including referrals and ACO care management
  • Experience navigating insurance payors and working in direct patient care/support
  • Experience working in a medical office environment
