POSITION OVERVIEW

Support the Case Management team with essential administrative functions related to post-acute care facilities. Responsible for managing the patient documentation process concerning transfers to post-acute care settings, including Medical Records, Core Measures, Medicare correspondence, authorizations, OBRA compliance, and various special initiatives. Aims to enhance the Long Term Care Department's efficiency through survey preparedness, maintaining physician visit logs, and daily data entry for the CMS Payroll-Based Journal (PBJ) Systems. Committed to delivering high-quality service that positively impacts the patient and family experience.

KEY RESPONSIBILITIES

  1. Facilitate placements in Post-Acute Care by:
    1. Managing paperwork via email and fax.
    2. Conducting outreach to post-acute care facilities and other transfer sites.
    3. Preparing and organizing Medical Records.
    4. Processing referrals, authorizations, and OBRA documentation.
    5. Coordinating transfer arrangements, including necessary paperwork.
  2. Serve as the admission liaison for Long Term Care placements, gathering data for screening and pre-admission evaluations, coordinating the admission of new residents, providing tours for prospective admissions, and maintaining a waitlist for Long Term Care.
  3. Assist patients in ensuring proper documentation is completed for Medicare letters.
  4. Act as a liaison between healthcare providers and various departments within the organization, as well as with external stakeholders.
  5. Perform administrative tasks such as typing, filing, tracking communications, and photocopying.
  6. Maintain an organized and tidy filing system and workspace.
  7. Ensure a clean, safe, and functional work environment.
  8. Engage in quality improvement initiatives.
  9. Participate in special projects as assigned.
  10. Input data daily into the CMS for the PBJ System, in compliance with the Affordable Care Act.
  11. Maintain the Long Term Care State/CMS Survey documentation.
  12. Manage visiting physician schedules for Long Term Care.
  13. Exhibit knowledge of patient safety protocols to ensure a secure environment and effective use of equipment, minimizing risks to patients.
  14. Demonstrate understanding of and adherence to the organization’s mission, vision, values, policies, procedures, and ethical standards.
  15. Perform additional related duties as required.
QUALIFICATIONS
  1. EDUCATION
    1. High School Diploma or equivalent required.
    2. Certification as a Medical Assistant or Nursing Assistant preferred.
  2. TRAINING & EXPERIENCE
    1. Proficient in general office practices with strong communication, organizational, customer service, and interpersonal skills. Exceptional telephone etiquette is essential. Proficient in Microsoft Word, Excel, and Outlook.
  3. JOB KNOWLEDGE
    1. Familiarity with medical terminology is required.
PHYSICAL REQUIREMENTS
  1. Ability to travel independently within all healthcare facilities.
  2. Proficient motor skills for operating modern office equipment.
  3. Capability to stand for extended periods.
  4. Ability to navigate confined or crowded spaces.
  5. Capacity to sit for prolonged durations.
  6. Ability to perform reaching, bending, and lifting tasks for file access and supply retrieval.
