About the Role

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Right at Home. As a Care Coordinator, you will play a vital role in creating extraordinary client and family experiences, managing care plans, and ensuring the efficient delivery of services to our clients.

Key Responsibilities
  • Care Coordination
    • Develop and maintain comprehensive care plans for clients, ensuring continuity of care and effective communication with clients, families, care partners, and referral sources.
    • Collaborate with caregivers and support staff to develop and implement care plans, providing guidance and support as needed.
    • Coordinate services, treatments, and referrals based on prescribed care plans, ensuring safe, appropriate, and meaningful care is provided.
    • Regularly follow up with clients to monitor progress, provide support, and make necessary adjustments to care plans.
  • Communication and Documentation
    • Maintain accurate and up-to-date records of client information, shifts, and care plans in our electronic health record system.
    • Facilitate communication between caregivers, clients, and their families to ensure everyone is informed about shift details and care instructions.
    • Answer inquiries and provide information about home care services, appointment availability, and general home care procedures.
    • Assist with recruiting, interviewing, and onboarding new employees as assigned.
  • Problem Solving
    • Address scheduling conflicts, cancellations, and rescheduling requests promptly and efficiently.
    • Identify and resolve potential scheduling bottlenecks or issues that may impact client care.
    • Maintain a calm, positive demeanor at all times and effectively multitask in a fast-paced office environment.
  • Training and Support
    • Provide guidance and training to new employees or team members on scheduling protocols and best practices.
    • Assist colleagues in handling complex care and scheduling scenarios and resolving issues.
    Requirements
    • High school diploma or equivalent; Associate's or Bachelor's degree preferred.
    • Previous experience in home care administration, appointment scheduling, or care coordination is preferred.
    • Familiarity with home care terminology and procedures.
    • Proficiency in using Google Workspace applications.
    • Excellent organizational and time management skills with the ability to multitask and prioritize tasks effectively.
    • Strong communication and interpersonal skills to interact with clients, caregivers, and support staff.
    • Attention to detail and accuracy in managing client records and scheduling information.
    • Empathy and compassion in dealing with clients and their families during challenging times.
    • Ability to work independently and as part of a team in a fast-paced home care environment.
