POSITION SUMMARY:

This role is essential in delivering options counseling, resource referrals, and case management services for the Aging and Disability Resources for Colorado (ADRC). The position aids clients with specific applications and fosters partnerships with community agencies, organizations, businesses, and individuals to expand the reach of ADRC services. Additionally, this role serves as the initial referral entry point for the Colorado Transitions Coordination program.

This is a full-time position with a schedule of Monday through Friday, from 8 AM to 5 PM.



KEY RESPONSIBILITIES:


- Respond to the ADRC main line during business hours, assessing callers' needs and providing suitable referrals to community resources.

- Conduct meetings with individuals experiencing physical, mental, and/or cognitive disabilities, employing person-centered strategies to help them understand the benefits and limitations of long-term services and support options.

- Maintain comprehensive knowledge of long-term care services available in the community and assist clients in completing applications for necessary programs.

- Ensure timely and accurate record-keeping in the State's database (SUDS), including log notes, assessments, care plans, and other relevant documentation.

- Manage pertinent documents such as release of information forms and status reports.

- Provide short-term case management for clients utilizing ADRC voucher services and long-term case management for up to 10 cases within the ADRC's Home Connections program.

- Oversee financial responsibilities associated with services approved under the voucher services and Home Connections program, including processing payments and communicating with service providers.

- Assist in managing and supervising ADRC's Supporting Our Seniors (SOS) volunteers, including scheduling, documentation, and addressing volunteer concerns.

- Serve as the initial point of entry for the Colorado Transitions Coordination Program, offering options counseling and information to nursing home residents interested in enrollment.

- Engage in data collection, ongoing training, and community collaboration, maintaining reports for the ADRC program and relevant funding agencies.

- Collaborate with community partners to ensure the ongoing success of the ADRC and actively seek resources to meet client needs.

- Attend community meetings and participate in committees and coalitions, delivering presentations to various community agencies and groups.

- Facilitate evidence-based programming by coordinating training and classes with local agencies and participants.
