Position Overview: The Bilingual Care Coordination Nurse plays a crucial role in collaborating with healthcare professionals to enhance the well-being of patients with chronic illnesses or intricate healthcare needs.

Key Responsibilities:

  • Conduct thorough evaluations of patients' health requirements, including their medical status, functional abilities, psychosocial circumstances, and available support networks to identify potential needs.
  • Develop and implement care strategies in partnership with primary care physicians and the healthcare team.
  • Deliver health education to patients and their families to empower them in managing their healthcare.
  • Identify patients at risk for early intervention.
  • Utilize data analysis to pinpoint at-risk patients and communicate findings to the care team.
  • Refer patients to various resources such as nutrition, social services, rehabilitation, mental health specialists, diabetes education, and wellness workshops.
  • Engage with patients who have utilized emergency services or have been hospitalized post-discharge to ascertain the reasons for their visits and collaborate on strategies to prevent future occurrences.
  • Coordinate care for patients with complex cases or those seeing multiple specialists.
  • Maintain precise and current documentation of patient assessments and care plans to ensure seamless integration of information for the healthcare team, thereby supporting ongoing quality care in line with evidence-based practices.
  • Analyze trends in patient care and work closely with the care team to enhance overall care quality and efficiency.
  • Leverage registry data to identify service gaps and initiate appropriate interventions.
  • Lead and participate in patient care meetings.
  • Collaborate with physician office staff to manage patient panels and assess outcomes.
  • Advocate for patient care issues to ensure that the quality and type of care are tailored to meet the unique needs of each patient and their family.
  • Engage in committees and initiatives related to the development of Medical Homes, Accountable Care Organizations (ACO), and other insurance programs.

Qualifications:

  • Associate's Degree in Nursing (ADN/ASN)
  • Bachelor's Degree (BS/BA/BSN) preferred
  • Previous experience in care coordination within a community or primary care setting
  • Experience in program development, operations management, or supervisory roles

Physical Requirements: The role necessitates the ability to see and read information, labels, and assess patient needs, operate medical equipment, and communicate effectively with patients and healthcare providers. Frequent computer use for documentation and information access is required, along with the ability to lift and transport medical supplies and equipment.
