POSITION SUMMARY:


This role is pivotal in delivering options counseling, resource referral, and case management services through our Aging and Disability Resources for Colorado (ADRC). The position aids clients with specific applications and fosters partnerships with community agencies, organizations, businesses, and individuals to enhance ADRC's outreach. Additionally, this role serves as the initial referral point for the Colorado Transitions Coordination program.

This is a full-time position with a schedule from Monday to Friday, 8 AM to 5 PM.

The starting wage for this position is competitive.

KEY RESPONSIBILITIES:


- Respond to the ADRC main line during business hours, assessing callers' needs and providing suitable referrals to community resources.

- Conduct meetings with individuals facing physical, mental, and/or cognitive disabilities, utilizing person-centered approaches to help them understand the benefits and limitations of long-term services and support options; connect them to necessary services and act as a liaison with service providers.

- Maintain comprehensive knowledge of long-term care services available in the community. Assist clients in completing applications for essential programs. Follow up with clients as needed to ensure service connections and delivery.

- Keep accurate and timely records in the State's database (SUDS), including log notes, assessments, care plans, and other relevant documentation.

Manage other important documents such as release of information forms and status reports.

- Provide short-term case management for clients utilizing ADRC voucher services and long-term case management for up to 10 cases within the ADRC's Home Connections program. Responsible for the financial aspects of services approved under the voucher services and Home Connections program, including making credit card purchases, completing check requests, and ensuring proper coding and financial procedures for provider payments.

- Assist in managing and supervising ADRC's Supporting Our Seniors (SOS) volunteers, including scheduling, maintaining documentation, addressing volunteer concerns, and providing recognition.

- Serve as the initial entry point for the Colorado Transitions Coordination Program, offering Options Counseling and information to nursing home residents interested in this program.

Ongoing data collection, training, and community collaboration are also required. Maintain reports for the ADRC program and relevant funding agencies.

- Update tracking sheets for ADRC monthly reports, including mileage, training hours, community outreach, Medicaid applications, and monthly surveys. Collaborate with community partners to ensure the ongoing success of the ADRC.

Seek out resources to meet client needs. Attend community meetings and participate in committees and coalitions. Provide presentations to various community agencies and groups. Facilitate evidence-based programming, including training and coordinating classes with local agencies and participants.
