PeaceHealth Seeks Hospice Care Specialist

We are seeking a skilled Hospice Care Specialist to join our team. This is a Per Diem/Relief, 0.00 FTE, Day position with a salary range of $36,550 - $54,830.

About the Role

The successful candidate will be responsible for providing direct social work services in Home Health and/or Hospice. Key duties include:

  • Providing casework services to clients related to their medical/psychiatric diagnosis.
  • Performing holistic evaluations of patient/home environments.
  • Developing and implementing plans of care.
  • Coordinating with healthcare teams to ensure smooth care transitions.

We require a Master's Degree in Social Work from an accredited institution. A Washington State Advanced License or equivalent is also necessary.

Requirements
  • Master's Degree in Social Work from an accredited institution.
  • Washington State Advanced License or equivalent.
  • Basic Life Support certification.
  • Valid driver's license and auto insurance.
