Job Description

As a Clinical Pharmacy Professional - Hospice Care Specialist, you will play a critical role in our hospice care team. Your primary responsibilities will include:

  • Providing clinical consultative services to patients and therapeutic recommendations to nurses, physicians, and other hospice personnel.
  • Conducting and evaluating medication profiles/histories, assessing compliance, and suggesting modifications to optimize medication regimens using sound clinical pharmacy principles.
  • Answering customer questions and helping resolve customer issues in a timely and professional manner.
  • Entering, accessing, and retrieving data in the pharmacy system.
  • Transcribing, verifying, and filling medication orders.
  • Maintaining proper storage and security conditions for medications.
  • Contributing to the maintenance of a productive and error-free work environment by keeping pharmacy workspaces neat and orderly.

Required Skills and Qualifications

To be successful in this role, you will need to have:

  • Exceptionally strong written and verbal communication skills, with a special emphasis on effective telephone correspondence with a customer-first attitude.
  • Excellent problem-solving skills, with a demonstrated ability to prioritize.
  • The ability to work with minimal supervision and maintain courteous and professional composure during stressful situations.
  • The ability to work cooperatively with both pharmacists and technicians.
