Job Summary

The Director of Care Coordination is a key leadership position responsible for the strategic development and execution of care coordination across the healthcare system. This role requires a strong clinical background, experience in care coordination, discharge planning, and case management.

Key Responsibilities
  • Develop and implement a comprehensive strategic plan for care coordination, taking into account internal and external factors, industry best practices, and institutional goals.
  • Collaborate with clinical leadership to provide expertise and support to improve performance in payor designated quality outcomes and contractual obligations related to care coordination.
  • Partner with clinical leadership and CCE to develop tools that display actionable critical outcome factors, focusing on value (quality/cost) including utilization and quality metrics.
  • Review findings with the Executive Team and provide feedback and support for redeployment of resources and workflows to optimize care coordination.
  • Establish policies, procedures, and protocols for departmental functions, ensuring compliance with CMS and other regulations.
  • Develop processes to anticipate and address specific situations within the department, including long stay patients, post-acute resource referral lists, denials, and termination of benefits.
  • Provide timely and effective departmental and organizational communication.
Requirements
  • Master's Degree in Nursing, Business Administration, Health Administration, Social Work, or related field required.
  • Five years of progressive experience in Care Coordination leadership required. Strong clinical background, experience in care coordination, discharge planning, and overall case management experience preferred.
  • Valid Nursing or LICSW credentials required. Nationally recognized care management certification preferred.
