We are seeking a skilled and compassionate Nurse Care Coordinator to join our team at Elliot Health. As a Nurse Care Coordinator, you will play a vital role in delivering high-quality care to patients and families across the lifespan, from newborns to older adults.
Key Responsibilities:
- Deliver care to patients and families with needs ranging from basic to complex, utilizing the nursing process.
- Create a therapeutic and caring environment for patients and families, promoting optimal patient outcomes and an effective professional practice environment.
- Embrace and promote change to support optimal patient outcomes and an effective professional practice environment.
- Involves patients/families in goal setting and planning, and communicates that plan of care to patients and families for input, and to colleagues to ensure seamless care across shifts and settings.
- Demonstrate accountability for continued professional growth and development, and for own role in smooth operations of the unit.
- Recognize and manage rapidly changing situations, utilizing current evidence and relevant national/specialty standards to guide practice and/or performance.
- Demonstrate competent clinical practice, utilizing clinical decision making and critical thinking skills.
Requirements:
- Graduate of an accredited school of nursing, Bachelor of Science in Nursing preferred.
- New Hampshire Registered Nurse or Compact State Registered Nurse License.
- Three (3) years nursing experience preferred, of which 2 or more should be related to pediatrics (hospital or community setting), NICU, PICU, or pediatric oncology.
- Ability to obtain BLS qualification during orientation period and maintain during employment.
- Nurses are expected to pursue professional certification upon eligibility or within three years of entering the specialty practice.
