We are seeking a skilled and compassionate Registered Nurse (RN) to serve as a Hospice Case Manager in our care team. The ideal candidate will have one year of nursing experience and a current RN license specific to the state(s) you are assigned to work.

Responsibilities:
  • Develop individualized patient care progress records with accuracy and timeliness.
  • Collaborate with the care team to develop and implement a plan of care.
