Job Requirements

To succeed in this role, you'll need a Bachelor's or Master's degree in social work and two years of experience in social work within mental health or medical settings.

Position Details

">

We're currently seeking a full-time weekend Social Worker (30 hours) for our Burlington Hospice Facility. The position involves providing intervention and assistance to the Social Work Department, consistent with generalist social work practice. Your responsibilities will include:

  • Resource assessment and planning
  • Intervention
  • Evaluation
  • Case management
  • Information and referral
  • Supportive presence and counseling
  • Advocacy
  • Community resource access for clients
  • Long-term care planning, including LTC placement
  • Completing necessary documentation, such as FL-2s, and obtaining PASRRs

You must be able to travel to, enter, and function effectively in patients' homes within our service area.

Education and Qualifications

To excel in this role, you'll need:

  • Bachelor's or Master's degree in social work
  • Two years of experience in social work within mental health or medical settings
  • The ability to travel, enter, and function effectively in patients' homes
  • Good judgment, concentration, and ability to follow procedures
  • Effective communication skills with diverse backgrounds and educational levels
  • Ability to work independently and with frequent interruptions
  • A valid state-issued driver's license and automobile liability insurance

Estimated Salary Range

">

$55,000 - $65,000 per year, based on experience and qualifications.
