Hospice Support Coordinator

About the Role:
We are seeking an experienced Hospice Support Coordinator to join our team at AuthoraCare Collective. In this full-time weekend position, you will provide crucial support to patients and their families on our Inpatient Unit at our Burlington Hospice Facility.
Responsibilities:
- Assess patient needs and develop tailored support plans
- Coordinate with healthcare teams to ensure comprehensive patient care
- Provide emotional support and counseling to patients and families
- Manage case files and maintain accurate records
- Collaborate with interdisciplinary teams to optimize patient outcomes
Requirements:
- Bachelor's or Master's degree in social work required
- Two years of experience in social work, preferably in mental health or medical settings
- Valid driver's license and reliable transportation
- Ability to travel to patients' homes within the service area
What We Offer:
- A competitive estimated salary range of $65,000 - $85,000 per year, depending on experience
