Compassionate Home Health Nurse Specialist

We are seeking a skilled Home Health RN Case Manager to join our team at Gentiva Hospice. As a dedicated healthcare professional, you will be responsible for providing high-quality care to patients in their homes, working closely with physicians, family members, and other healthcare professionals.
About the RoleThis is a full-time position, Monday through Friday, days, with a competitive salary of $**85,000 - 110,000 per year**, depending on experience. Our Home Health RN Case Managers are eligible for a comprehensive benefits package, including medical, dental, vision plans, wellness programs, paid time off, and career growth opportunities.
As a Home Health RN Case Manager, you will:
- Administer skilled nursing care to clients requiring professional services in their homes
- Teach and supervise the family and other members of the nursing team
- Act as primary liaison between physician, patient/family and home health team
- Provide direct patient care services to patients and supportive care to the patient's family
To be successful in this role, you will need:
- RN license in the state of requested employment (or eligibility to obtain)
- 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred
- Current driver's license and automobile insurance
- Current CPR certification
