Hospice Nurse Care Coordinator

We are seeking an experienced Hospice Registered Nurse (RN) to join our team in Burlington, providing per diem services. As a member of our hospice team, you will be responsible for delivering patient-centered care, working closely with families, patients, and our interdisciplinary team to develop and implement personalized care plans.
The ideal candidate will possess a strong foundation in nursing theories and concepts, with a minimum of two years of clinical nursing experience and at least one year of hospice nursing experience. You will have a deep understanding of the principles and practices of nursing, including the ability to communicate effectively with patients, families, and healthcare professionals.
In this role, you will be required to:
- Apply your knowledge of nursing theories and concepts to deliver high-quality patient care
- Coordinate physical care and documentation with family, patient, physician, and hospice team members
- Communicate effectively with patients, families, and healthcare professionals at all levels
- Participate in interdisciplinary team meetings and quality reviews as scheduled by the agency
To be successful in this role, you must possess:
- A current RN licensure in the state of practice
- Two years of clinical nursing experience, with at least one year of hospice nursing experience preferred
- Knowledge of home health and hospice care programs, including the nurse's role on the patient care team
- Working knowledge of nursing practice act, agency policies and procedures, and reimbursement guidelines
- CPR certification and 12 hours of continuing education annually
