Senior Clinical Liaison for Hospice Services

We are seeking an experienced Registered Nurse to fill the role of Senior Clinical Liaison for Hospice Services at Hospice & Palliative Care of Greensboro. This is a full-time position with weekend responsibility/rotation, supporting our Burlington campus/service areas.

Job Summary:

The ideal candidate will possess excellent assessment, clinical nursing, and coordination skills, as well as effective interpersonal and communication skills. They will work closely with patients, caregivers, and healthcare providers to establish patient/caregiver care goals and implement interventions. The successful candidate will also communicate effectively with other members of the interdisciplinary team and health care providers, as needed.

About the Role:
  • Evaluate the clinical needs of newly referred and hospitalized hospice patients
  • Identify and inform patients and families about appropriate resources
  • Work with patients/caregivers and providers to establish patient/caregiver care goals and identify interventions
  • Ensure a medical plan of care is established and implemented for patients
Requirements:

To be considered for this role, applicants must meet the following requirements:

  • Must be currently licensed as an RN in North Carolina with a minimum of 3-5 years of experience in an acute or home health setting
  • Bachelor's degree in nursing and Hospice experience preferred
  • RN Certification in Hospice and Palliative Care preferred
  • Valid state-issued driver's license required
  • Must carry automobile liability insurance at limits required by agency
  • Must have own transportation
