Direct Support Professional

We are seeking a highly motivated and compassionate Direct Support Professional to join our team at YAI. As a Direct Support Professional, you will play a vital role in supporting individuals with intellectual and developmental disabilities to live, learn, and thrive in their communities.
Key Responsibilities- Provide direct care services to individuals with intellectual and developmental disabilities, including counseling, training in daily living skills, and assistance with personal care and medical needs.
- Ensure the health, safety, and wellness of the individuals we support, and actively promote a culture of person-centered care.
- Develop and implement individualized plans to support individuals in achieving their goals and increasing their independence.
- Collaborate with interdisciplinary teams to ensure comprehensive support and services are provided to individuals.
- Maintain accurate and timely documentation of services provided, including progress notes and incident reports.
- Participate in ongoing training and professional development to enhance skills and knowledge in supporting individuals with intellectual and developmental disabilities.
- Escort individuals on community outings and activities, and provide support in daily living tasks such as grocery shopping and personal finance management.
- High School Diploma or equivalent required; experience working with individuals with intellectual and developmental disabilities preferred.
- Valid New York State driver's license and reliable transportation required.
- Ability to work flexible hours, including evenings and weekends, as needed.
- Strong communication and interpersonal skills, with ability to work effectively with individuals, families, and interdisciplinary teams.
- Ability to maintain confidentiality and handle sensitive information with discretion.
