Job Summary

We are seeking a highly skilled and experienced Resident Care Manager to join our team at Beaverton Rehab & Specialty Care LLC. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and leadership to our nursing staff.

Key Responsibilities
  • Develop and Implement Care Plans: Collaborate with healthcare professionals to create personalized care plans for residents, ensuring their physical, emotional, and social needs are met.
  • Lead and Manage Nursing Staff: Supervise and mentor RNs, LPNs, and CNAs, providing guidance and support to ensure high-quality patient care.
  • Conduct Daily Rounds: Regularly visit residents to assess their needs, provide care, and ensure their comfort and well-being.
  • Review and Revise Care Plans: Continuously evaluate and update care plans to reflect changes in resident needs and preferences.
  • Collaborate with Healthcare Professionals: Work closely with physicians, therapists, and other healthcare professionals to ensure comprehensive and coordinated care.
  • Maintain Accurate Records: Document resident care, progress, and any concerns or issues in a timely and accurate manner.
Requirements
  • Nursing Degree: Possess a nursing degree from an accredited college or university.
  • Current RN License: Hold a current, unencumbered, active license to practice as an RN in this state.
  • Leadership Experience: Have a minimum of 1 year of experience as a supervisor in a healthcare setting.
  • Rehabilitative and Restorative Nursing Practices: Possess training in rehabilitative and restorative nursing practices.
  • Electronic Medical Records: Have experience with Electronic Medical Records and computer documentation systems.
