Job Summary

We are seeking a highly skilled and experienced Resident Care Manager to join our team at Beaverton Rehab & Specialty Care LLC. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and leadership to our nursing staff.

Key Responsibilities
  • Develop and Implement Care Plans: Collaborate with healthcare professionals to create personalized care plans for residents, ensuring their physical, emotional, and social needs are met.
  • Lead and Manage Nursing Staff: Supervise and mentor RNs, LPNs, and CNAs, providing guidance and support to ensure high-quality patient care.
  • Monitor and Evaluate Patient Care: Conduct regular rounds to assess patient care, identify areas for improvement, and implement changes as needed.
  • Communicate Effectively: Maintain open and transparent communication with patients, families, and healthcare team members to ensure seamless care coordination.
  • Stay Up-to-Date with Industry Developments: Participate in ongoing education and training to stay current with best practices, regulations, and industry advancements.
Requirements
  • Nursing Degree and Licensure: Possess a current, unencumbered RN license and a degree from an accredited college or university.
  • Leadership Experience: Minimum 1 year of experience as a supervisor in a healthcare setting, with a proven track record of leadership and patient care expertise.
  • Rehabilitative and Restorative Nursing Practices: Knowledgeable of nursing and medical practices, as well as laws, regulations, and guidelines that pertain to nursing care facilities.
  • Electronic Medical Records and Computer Documentation: Experienced with EMRs and computer documentation systems.
