Job Overview

Position: Hospice Registered Nurse - Night Shift

Care Partners, a respected not-for-profit hospice organization in Oregon, is seeking a dedicated Registered Nurse with a passion for providing compassionate end-of-life care. With over 40 years of experience, we are committed to delivering holistic services to our patients and their families throughout the Portland Metropolitan area.

This role involves working directly with patients in various settings, including private homes and assisted living facilities. Our team is known for going above and beyond to ensure families feel supported during this critical time.

Key Responsibilities:

  • Conduct patient visits and provide telephone support for symptom management and urgent needs during night hours.
  • Deliver comprehensive care following established care plans across diverse environments.
  • Assist in patient assessments under the guidance of a Registered Nurse, identifying any changes in patient condition.
  • Coordinate with physicians to obtain necessary orders and follow up on lab results.
  • Monitor patient responses to treatments and medications, ensuring adherence to safety protocols.
  • Document and report both objective and subjective patient data accurately.
  • Utilize electronic medical records for effective documentation and care planning.
  • Foster strong relationships with patients, families, and team members, promoting an inclusive environment.

Qualifications:

  • Graduate from an accredited nursing program.
  • Current, unencumbered RN license in Oregon.
  • Minimum of one year of nursing experience, preferably in hospice, acute care, or home health settings.
  • Basic Life Support (BLS) certification required prior to patient contact.
  • Reliable transportation and valid driver's license.
  • Strong organizational skills and attention to detail.
  • Excellent communication abilities.
  • Experience working independently with minimal supervision.
