Key Responsibilities:
- Review and assess patient care plans to ensure appropriate goals, issues, strategies, and modifications based on nursing needs.
- Ensure all staff involved in patient care are informed of the care plans.
- Organize and lead care plan meetings.
- Oversee and guide the completion of assessments.
- Manage the Utilization Review (UR) process effectively.
- Supervise treatment records, flow sheets, and physician orders.
- Participate in facility evaluations by authorized regulatory bodies.
- Must hold a nursing degree from an accredited institution.
- Must possess a current, unencumbered, active nursing license.
- MDS Certification is preferred.
- Familiarity with the RAI process, CAAs, RUG categories, and MDS 3.0 is essential.
- Knowledge of nursing and medical practices, regulations, and guidelines relevant to nursing care facilities.
- Ability to document nursing notes in a clear and detailed manner reflecting the care provided and the resident's response.
- Experience with Electronic Medical Records and documentation systems.
- Maintain confidentiality of all resident care information in accordance with HIPAA regulations.
