Assessment Specialist

**About the Role**
We are seeking a highly motivated and organized Disability Support Coordinator to join our team as a Coordinated Assessment System (CAS) Assessor. In this role, you will work closely with Care Coordination Organizations (CCOs) to conduct assessments, schedule appointments, and ensure timely completion of assessments.
As a CAS Assessor, you will be responsible for conducting outreach to CCOs, gathering essential consumer information, and informing necessary parties about upcoming assessments. You will also schedule assessment appointments, meet productivity targets, and document outreach activities for tracking and reporting purposes.

**Responsibilities:**
- Conduct outreach to CCOs to gather essential consumer information and inform necessary parties about upcoming assessments.
- Schedule assessment appointments and meet productivity targets.
- Document outreach activities and ensure assessments are assigned to the most appropriate resources to meet consumer needs.
- Monitor and improve the scheduling process to ensure efficiency and alignment with program goals.
