Job Summary:

We are seeking an experienced nurse to conduct in-home assessments and determine eligibility for community-based long-term care services.

Responsibilities:

  • Conduct thorough patient assessments to identify needs for community-based services.
  • Review patient medical history, including current medications and relevant medical conditions.
  • Develop and implement care plans to meet patient needs.

Requirements:

  • 3+ years of experience in managed care programs.
  • ~5+ years of experience in care/case management, disease management, or population health management.
  • Bilingual.
  • Prior field experience.

  • Estimated Salary: $85,000 - $110,000 per year.
