Job Title: Assessment Nurse - Healthcare Specialist

About the Role:

  • We are seeking an experienced and skilled Assessment Nurse to join our team in Bronx, NY.
  • This per diem position involves conducting field assessments, determining enrollment and continued plan eligibility, and coordinating with the Care Management Team.
  • You will work closely with patients, families, and caregivers to understand their needs and provide personalized care.

Responsibilities:

  • Conduct home assessments and re-assessments using the Uniform Assessment System (UAS)
  • Determine service needs and make recommendations for adjustments
  • Liaise between members, families, and the Care Management Team
  • Provide training and education on self-care techniques and prevention strategies
  • Collaborate with healthcare agencies and contract nurses to ensure high-quality care

Requirements:

  • New York State RN license
  • Ability to travel to Bronx, NY
  • Work experience in geriatrics, home care, or discharge planning preferred

Hiring Range:$81,099 - $116,480
