About the Role: As a Resident Assessment Specialist at Inspir Carnegie Hill, you will play a critical role in ensuring that our residents receive the highest quality of care. Your primary responsibility will be to conduct comprehensive evaluations of prospective residents and current residents, assessing their needs and developing personalized care plans.

Key Responsibilities:
  • Conducting comprehensive evaluations of prospective residents and current residents.
  • Collaborating with ICT at the community to develop individualized service plans based on evaluation findings.
  • Providing guidance and support to residents and their families regarding available services and resources.
  • Maintaining resident records in compliance with regulatory standards.

Requirements:
  • A baccalaureate degree in nursing and a minimum of two years of full-time clinical experience in nursing.
  • Licensure to practice as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of New York.
  • Ability to travel around downstate New York, including the 5 boroughs, Long Island, Westchester, and parts of New Jersey.
