About the Role

We're looking for a dedicated and compassionate RN with a minimum of 2 years of clinical experience, preferably in-home care. As a Home Assessment Specialist, you'll conduct thorough assessments in the comfort of clients' homes, using the UAS-NY assessment tool to determine their eligibility for community-based long-term services.

You'll be working closely with the clinical leadership team to ensure accurate and timely completion of assessments. Your ability to establish positive relationships with clients, caregivers, and facility staff will be essential in delivering exceptional patient care.

Key Responsibilities
  • Conduct in-home, in-facility, or telehealth assessments using the UAS-NY assessment tool
  • Evaluate clients' health status, strengths, care needs, and preferences
  • Establish and maintain positive relationships with clients, caregivers, and facility staff
  • Accurately document medical information and concerns
Requirements
  • Active RN license in the state of New York
  • Minimum 2 years of clinical experience, preferably in-home care
  • Ability to travel locally and work in a fast-paced environment
