Job Description

In this role, you will be responsible for conducting assessments to determine service needs and continued eligibility. You will work closely with our care management team to maintain efficient and effective individualized care management operations.

Responsibilities
  • Provide weekly work availability to our coordinator.
  • Notify our coordinator promptly of scheduling delays or conflicts.
  • Schedule appointments within 24 hours of case assignment for all scheduled appointments.
  • The assessment process must be completed within 7 days of case assignment.
Requirements
  • A Physical within one year with a habituation statement on form or drug screen done within 6 months.
  • Rubella, Rubeola, PPD/QuantiFERON within one year with chest x-ray if positive dated after positive result.
  • HEP B Form in HR paperwork.
  • Annual self-health assessment, TB Screen, Flu shot or declination.
Qualifications
  • Nursing Registration, Nursing License, Malpractice Insurance.
  • CCM Certification (Preferred), UAS Certification (Required), 6 months exp with UAS work minimum.
  • Tech Savvy, Microsoft Office, Lenavi Pro software used by the UAS Nurses.

Estimated Salary: $100-$160 per hour based on experience and location in Manhattan.

This is a per diem position requiring onsite assessments. We offer competitive rates and benefits to qualified candidates.
