Position Overview: The PRN Speech Language Pathologist in Home Health is tasked with the comprehensive assessment and evaluation of patient care requirements related to addressing speech and language challenges, as well as functional training in communication, swallowing, and cognitive difficulties.

Responsibilities:

  1. Conduct thorough assessments to identify patient needs and formulate a tailored treatment plan.
  2. Implement interventions designed to enhance the patient's overall well-being and monitor their progress.
  3. Deliver services in accordance with state regulations governing speech therapy practices, aligning with the established care plan and collaborating with other healthcare professionals.
  4. Evaluate the patient's functional capabilities and therapy requirements using standardized assessment tools.
  5. Collaborate with physicians to develop and refine the therapy plan of care.
  6. Address various aspects of speech including production, fluency, language, cognition, voice, resonance, swallowing, and auditory rehabilitation using evidence-based practices.
  7. Document and communicate the patient's response to treatment and any changes in their condition to the supervising nurse or physician.
  8. Educate patients, families, caregivers, and healthcare team members on relevant therapy areas within the scope of speech-language pathology.
  9. Travel to visit patients across the designated service area and other affiliated agencies as necessary.
  10. Complete progress notes for each visit in accordance with agency policies.
  11. Finalize all patient evaluations and develop the speech therapy plan of care following state-specific guidelines.
  12. Report evaluation outcomes, goals, and anticipated care frequency to the supervising nurse promptly.
  13. Engage in staff meetings and committees as requested by the agency.
  14. Provide in-service training as needed by agency leadership.
  15. Prepare written discharge summaries and condition reports at regular intervals.
  16. Adhere to established Therapy Standards of Practice in delivering services to all patients.
  17. Commit to ongoing education to maintain and enhance clinical expertise and ensure compliance with state licensure requirements.
  18. Participate in weekend rotations to provide services as required.

Licensure Requirements:

  • Must hold a valid Speech Therapy license in the state.
  • Current CPR certification is required.
  • Possession of a valid driver's license and vehicle insurance, along with access to reliable transportation.
