Licensed Speech Language Pathologist

The PRN Speech Language Pathologist in Home Health is tasked with assessing and evaluating patient care requirements related to addressing speech and language disorders, as well as providing functional training in communication, swallowing, and cognitive challenges. Following this assessment, the Speech Language Pathologist collaborates to formulate a treatment strategy, implements interventions aimed at enhancing the patient's quality of life, and monitors the patient's progress.
Key Responsibilities
- Delivers services within the defined scope of practice as established by state regulations governing speech therapy, in alignment with the care plan, and in collaboration with other healthcare professionals.
- Assesses the patient's functional capabilities, evaluates all body systems as necessary for documentation, and identifies speech therapy requirements utilizing standardized and/or criterion-referenced assessments. Engages with the physician to develop the therapy care plan.
- Addresses patients' speech production, fluency, language, cognitive skills, voice, resonance, swallowing, and auditory habilitation/rehabilitation employing evidence-based practices.
- Records and communicates the patient's response to treatment and any changes in their condition to the supervising nurse and/or physician.
- Educates the patient, their family, and/or caregivers, as well as other healthcare team members, regarding therapy aspects within the speech-language pathology domain.
- Visits patients throughout the designated service area of their agency and other nearby LHC agencies as required.
- In accordance with the treatment plan, completes a progress note for each visit and submits documentation per LHC policy.
- Conducts all patient evaluations and formulates the speech therapy care plan adhering to state-specific regulations. Reports evaluation outcomes, goals, and anticipated frequency of care to the nurse supervisor within 24 hours of the evaluation.
- Engages in staff meetings and committees as requested by the agency.
- Provides in-service training as requested by agency leadership. Prepares a written discharge summary and/or a report on the patient's condition at least every 60 days.
- Complies with LHC Therapy Standards of Practice in delivering services to all patients.
- Takes responsibility for pursuing education to maintain and enhance clinical knowledge and skills in line with current standards of care for the profession and to uphold state licensure.
- Participates in a weekend rotation to provide services for their agency and other nearby LHC agencies as necessary.
Licensure Requirements
- Must hold a current license in Speech Therapy in the state.
- Current CPR certification is required.
- Must possess a valid driver's license and vehicle insurance, and have access to a reliable vehicle or public transportation.
