Job Overview

The PRN Speech Language Pathologist in Home Health is tasked with assessing and evaluating patient care requirements associated with addressing speech and language challenges, alongside functional training in communication, swallowing, and cognitive difficulties. Following this assessment, the Speech Language Pathologist collaborates to formulate a treatment strategy, implements interventions aimed at enhancing the patient's quality of life, and monitors the patient's progress.

Key Responsibilities
  • Delivers services within the professional scope as dictated by state regulations governing speech therapy, in alignment with the care plan, and in collaboration with other healthcare professionals.
  • Assesses the patient's functional capabilities, evaluates all bodily systems as mandated for documentation, and identifies speech therapy requirements utilizing standardized and/or criterion-referenced assessment tools. Engages with the physician to devise the therapy care plan.
  • Administers treatment for patients' speech production, fluency, language, cognition, voice, resonance, swallowing, and auditory habilitation/rehabilitation employing evidence-based practices.
  • Records and communicates the patient's response to treatment and any changes in their condition to the supervising nurse and/or physician.
  • Educates the patient, their family, and/or caregivers, as well as other healthcare team members, regarding therapy aspects within the speech-language pathology domain.
  • Conducts patient visits throughout the designated service area of their agency and other nearby LHC agencies as required.
  • Completes a progress note for each visit in accordance with the treatment plan and submits these notes following LHC policy.
  • Finalizes all patient evaluations and formulates the Speech Therapy plan of care adhering to state-specific regulations. Reports evaluation outcomes, objectives, and expected frequency of care to the nurse supervisor within 24 hours of the assessment.
  • Engages in staff meetings and committees as requested by the agency.
  • Provides in-service training as requested by agency leadership. Prepares a written discharge summary and/or a comprehensive report on the patient's condition at least every 60 days.
  • Complies with LHC Therapy Standards of Practice in delivering services to all patients.
  • Takes responsibility for pursuing education to maintain and enhance clinical knowledge and skills that align with current care standards for the profession and to uphold state-issued licensure.
  • Participates in a weekend rotation to deliver services for their respective agency and other nearby LHC agencies as necessary.
Qualifications

Licensure Requirements
  • Must hold a current license in Speech Therapy in the state.
  • Current CPR certification is required.
  • Must possess a valid driver's license and vehicle insurance, along with access to a reliable vehicle or public transportation.
