We are seeking a skilled Speech Therapist to join our team at Patient Care. Key Responsibilities- Assessment and Evaluation: Conduct thorough assessments and evaluations of patient care needs, including speech and language disorders, and functional training in communication, swallowing, and cognitive impairments.
- Treatment Planning: Develop and implement treatment plans to address patient care needs, in accordance with the plan of care and in coordination with other members of the healthcare team.
- Interventions: Provide interventions aimed at improving and enhancing patient well-being, including speech production, fluency, language, cognition, voice, resonance, swallowing, and auditory habilitation/rehabilitation.
- Progress Monitoring: Monitor patient progress and report to the nurse supervisor and/or physician, as required.
- Education and Training: Instruct patients, families, and caregivers in areas of therapy within the scope of speech-language pathology.
- Travel: Visit patients within the entire service area of our agency and other nearby agencies, as needed.
- Documentation: Complete progress notes and submit them per our agency's policy.
- State Requirements: Complete all patient evaluations and develop the ST plan of care within state-specific guidelines.
- Continuing Education: Participate in staff conferences and committees, as requested, and access education to maintain and enhance clinical knowledge and skills.
- Licensure: Must be currently licensed in Speech Therapy in the state of [insert state].
- CPR Certification: Current CPR certification is required.
- Driver's License: Must have a current driver's license and vehicle insurance, and access to a dependable vehicle or public transportation.
