Position Overview

We are seeking a committed **Family Support Systems Coordinator** to contribute to our mission at **Patriot Enterprises LLC**. This role is essential in delivering holistic assistance to families with unique needs, ensuring effective collaboration with both military and civilian organizations.

Key Responsibilities
  • Conduct outreach visits to provide vital support to military personnel and their families.
  • Partner with governmental bodies and community service providers to discover resources for respite care outside the EFMP.
  • Develop and maintain a comprehensive directory of support services and contacts for families with special requirements.
  • Facilitate regular support groups and educational workshops.
  • Present information sessions on available family support services to military personnel and their families.
  • Provide guidance and referral services to help families navigate available resources.
  • Collaborate closely with families to strengthen resilience and advocacy skills.
  • Ensure meticulous record-keeping in accordance with relevant regulations.
  • Assist families in transitioning smoothly when necessary.
  • Gather and evaluate data for program enhancement and reporting purposes.
  • Implement strategies to connect families with essential support services.
Qualifications and Skills
  • Knowledge of human services principles and case management methodologies.
  • Awareness of military, state, and federal regulations concerning medical, educational, and family support services.
  • Ability to effectively deliver non-clinical case management support.
  • Excellent communication and interpersonal abilities.
  • Certification as a Life Coach from a recognized institution.
  • Willingness to travel as required.
  • Proficiency in Microsoft Office Suite.
  • Eligibility to work in the United States with a clear background check.
Educational Requirements
  • A Bachelor's degree in social work, psychology, counseling, education, or a related discipline.
  • A minimum of two years of experience in family-focused case management or social work.
