Key Responsibilities- Collaborate with Primary Care Providers (PCP) and community resources to ensure a holistic approach to patient care, considering individual social and cultural factors.
- Evaluate patients for issues that can be addressed through community-based care management.
- Serve as a bridge between the PCP, local health authorities, social services, hospitals, and community organizations to coordinate necessary health services.
- Work with network providers to ensure optimal client management.
- Establish and nurture relationships with community service providers through collaboration and participation in community events.
- Assist patients in addressing their health concerns through referrals and communication with the healthcare team.
- Maintain accurate client records in the Electronic Health Record (EHR) system.
- Create and implement personalized care management plans for identified patients.
- Conduct follow-up and outreach services through various methods including home visits, phone calls, or mail.
- Educate patients and their families on the significance of effective medical care management and how to navigate the healthcare system.
- Provide information on disease management, medication adherence, and preventive care strategies.
- Ensure compliance with hospital discharge instructions for high-risk patients to maintain continuity of care.
- Act as a liaison to healthcare providers to promote the use of Evidence-Based Practices.
- Support providers in coordinating services for high-risk patients by implementing best practices.
- Develop and deliver healthcare education programs and training sessions.
- Advocate for patients to access services that enhance their health status.
- Review patient care plans to eliminate unnecessary services and manage costs effectively.
- Conduct audits and compile data to support disease management initiatives.
- Ensure patient and family confidentiality in accordance with HIPAA regulations.
- Perform additional duties as required.
- Degree in Nursing (RN or LPN) or Registered Medical Assistant (RMA).
- Preferred experience in direct patient care or managed care settings.
- Valid driver's license and personal vehicle for travel between locations.
- Experience with HEDIS Quality Measures.
- Strong communication and customer service skills.
- Proficiency in Microsoft Word, Excel, and PowerPoint.
- Ability to work independently while collaborating with team members.
- Self-motivated with strong prioritization skills and a commitment to improving processes.
