This role is essential in delivering extensive assistance to families with unique needs while collaborating with both military and civilian organizations.
Key Responsibilities- Conduct home assessments to provide vital support to military personnel and their families.
- Engage with governmental bodies and civilian service providers to pinpoint resources for non-EFMP respite care.
- Maintain an updated directory of support networks and contacts tailored for families with special needs.
- Facilitate regular support groups and educational workshops.
- Present information sessions on available EFMP services to military personnel and their families.
- Provide information and referral services to help families access suitable resources.
- Collaborate closely with families to strengthen resilience and self-advocacy capabilities.
- Ensure accurate record management in accordance with Army and DoD regulations.
- Assist families in navigating transitions as necessary.
- Gather and evaluate data for program enhancement and reporting purposes.
- Implement policies and procedures to connect families with essential support services.
- Knowledge of human services principles and case management methodologies.
- Familiarity with military, state, and federal regulations concerning medical, educational, and family support services.
- Ability to effectively deliver non-clinical case management services.
- Excellent communication and interpersonal abilities.
- Life Coach certification from the Certified Life Coach Institute.
- Willingness to travel as required.
- Proficiency in Microsoft Office Suite.
- Eligibility to work in the United States and a clear background check.
- Bachelor's degree in social work, psychology, counseling, education, or a related discipline.
- A minimum of two years of experience in family-focused case management or social work.
