Open Sky Community Services is dedicated to providing comprehensive support through Adult Community Clinical Services, a program funded by the Department of Mental Health. Our mission is to implement evidence-based practices within a clinically oriented framework.
We serve individuals diagnosed with mental health conditions or dual diagnoses, who live in various settings including their own residences, apartments, and communal living spaces.
Our focus is on facilitating transitions to greater independence, enhancing natural support systems, and empowering individuals to achieve their personal aspirations.
The teams consist of collaborative, multidisciplinary professionals who are committed to delivering compassionate care.
As Community Support Counselors, you will act as a vital link between clients, community organizations, and families, coordinating treatment efforts that encompass employment, education, and healthcare services.
Your role will involve fostering respectful partnerships, establishing appropriate boundaries, and maintaining a supportive presence for clients and their families.
Community Support Counselors inspire individuals to transcend perceived limitations by equipping them with essential tools for success.
Key Responsibilities Include:
Implementing interventions outlined in the Treatment Plan under the guidance of licensed clinical staff.
Facilitating activities that promote the development of meaningful roles and relationships within the community.
Providing direct support and coaching to assist individuals in social interactions, conducting both individual and group activities aimed at enhancing readiness for rehabilitation.
Ensuring individuals have the necessary financial resources by aiding in job searches or applications for benefits.
Participating in the On-Call rotation.
Qualifications:
A High School Diploma, GED, or equivalent is required.
A valid Driver's License, reliable transportation, and a satisfactory driving record are essential.
Dependability, eagerness to learn, and openness to new experiences are important traits.
