Position OverviewThe Clinical Transition Coordinator is responsible for conducting psychosocial evaluations, providing crisis intervention, and facilitating resource referrals to support discharge planning and adjustment to health challenges for patients and their families.
This role involves collaborating with patients, families, and the healthcare team to develop tailored discharge plans based on comprehensive needs assessments. The coordinator will also manage referrals to home care services, rehabilitation facilities, and community resources.
Additionally, the coordinator will oversee the care authorization process with insurance providers and coordinate hospitalization details with the Utilization Management department to ensure proper admission status.
Experience RequirementsA minimum of 3 to 5 years of experience in the healthcare sector is preferred.
Educational QualificationsA Master’s Degree in Social Work (MSW) or a licensed Registered Nurse (RN) with a Bachelor of Science in Nursing (BSN) is preferred.
Licensure and Certification RequirementsCurrent New Jersey Social Work License; LCSW is preferred.
Alternatively, a Current New Jersey RN License is acceptable.
Certifications from the American Case Management Association (ACM) or the Commission for Case Manager Certification (CCMC) are preferred.
