Job SummaryWe are seeking a skilled Patient Care Coordinator to join our team. The ideal candidate will possess excellent interpersonal skills, a strong ability to work well with diverse populations, and a demonstrated commitment to delivering high-quality patient care.
Key Responsibilities- Bridge the gap between clinicians and patients by providing individualized support and assistance in navigating the healthcare system to ensure continuity of care.
- Collaborate with the RN Navigator disease team to provide clinical support, including scheduling, pre-certification processes, follow-up calls, and record retrieval for timely appointments.
- Establish relationships with patients, families, and healthcare providers to ensure seamless transitions of care.
- Work closely with the CPN, Navigators, Social Work, Dieticians, Call Center, Providers, Radiology scheduling, Chemo scheduling, and phone triage teams to support patient needs.
- Participate in daily multidisciplinary rounding on the inpatient oncology unit to identify needs and opportunities for coordination of care, follow-up, and scheduling.
- Act as a member of the Transition of Care team, assisting patients in transitioning from inpatient to outpatient areas without interruption in services and care.
- Connect patients with services for support, such as social work and financial counseling.
- Track patient contacts, follow-ups, barriers to care, and outcomes.
- Participate in interdepartmental and ancillary activities to ensure quality, cost-effective patient care.
- Experience working in a healthcare office setting or community outreach and education.
- Knowledge of healthcare, inpatient/outpatient settings, and oncology-specific patient populations.
- Strong interpersonal skills, demonstrated ability to work well with people of various backgrounds, ethnicities, and life experiences.
- Ability to give/take direction, follow through with tasks, and work independently.
- Flexibility, optimism, and a cooperative working style.
High School Diploma or Equivalent; clinical training (i.e., MA, CNA, LPN) required.
Special RequirementsOther Qualifications:
Fluency in Spanish a plus.
Competent in basic computer skills; knowledge of using an electronic health records system preferred.
