Serve as a vital link between healthcare providers and patients by offering personalized support and guidance in navigating the healthcare landscape to ensure seamless continuity of care.
Collaborate with the existing RN Navigator disease team to deliver clinical assistance related to appointment scheduling, pre-certification processes, follow-up communications, and timely record retrieval.
Engage with new patients within 24 hours of their visit to assess their follow-up needs and appointments. All clinical interactions will be documented in EPIC to facilitate care coordination, scheduling, and referrals to necessary resources.Work alongside CPN, Navigators, Social Workers, Dieticians, Call Center staff, Providers, Radiology scheduling, chemotherapy scheduling, and phone triage to address patients' requirements.
Participate in daily multidisciplinary rounds on the inpatient oncology unit to identify needs and opportunities for care coordination, follow-up, and scheduling.
All documentation will be recorded in EPIC to ensure effective communication across all clinical teams.Act as a key member of the Transition of Care team, assisting patients in moving from inpatient to outpatient settings without disruption in services and care.
Facilitate connections for patients to support services (e.g., social work, financial counseling).Maintain records of all patient interactions, follow-ups, barriers to care, and outcomes.
Experience Required
Experience in a healthcare office environment or community outreach and education is essential.
Familiarity with healthcare systems, inpatient/outpatient environments, and oncology-specific patient populations is preferred.
Strong interpersonal skills; proven ability to collaborate effectively with individuals from diverse backgrounds, ethnicities, and life experiences.
Capacity to receive and provide direction, complete tasks independently, and follow through on responsibilities.
Demonstrated flexibility, optimism, and a collaborative work style.
Education Requirements
Graduate of an NLN accredited School of Nursing.
Special Requirements Other Qualifications:
Fluency in Spanish is advantageous.
Proficient in basic computer skills; familiarity with electronic health records systems is preferred.
