Position Overview
The Senior Patient Care Coordinator will serve as a vital link between patients and healthcare providers, offering personalized assistance in navigating the healthcare landscape to ensure seamless continuity of care. Key responsibilities include:
  • Collaborating with the RN Navigator and disease management teams to provide clinical support, including scheduling, pre-certification processes, follow-up communications, and record retrieval for timely appointments.
  • Conducting outreach to new patients within 24 hours post-visit to assess follow-up needs and ensure appropriate care coordination.
  • Documenting all patient interactions and care coordination activities in the electronic health record system to facilitate communication among clinical teams.
  • Participating in daily multidisciplinary rounds in the inpatient oncology unit to identify patient needs and enhance care coordination.
  • Assisting patients in transitioning from inpatient to outpatient care, ensuring uninterrupted services and support.
  • Connecting patients with additional resources, such as social work and financial counseling services.
  • Tracking patient contacts, follow-ups, barriers to care, and outcomes to improve service delivery.
  • Engaging in interdepartmental activities to uphold quality and cost-effective patient care.
  • Providing feedback on the performance of the patient navigator team and managing time-off requests.
  • Leading the patient navigator team while upholding Cooper's core values.

Qualifications
  • Experience in a healthcare office environment or community outreach and education is essential.
  • Knowledge of healthcare systems, particularly in inpatient and outpatient settings, with a focus on oncology patient populations.
  • Strong interpersonal skills and the ability to collaborate effectively with diverse individuals.
  • Ability to follow directions, complete tasks independently, and maintain a flexible and optimistic approach.

Education Requirements
Graduate of an NLN accredited School of Nursing.

Additional Qualifications
Fluency in Spanish is a plus. Proficiency in basic computer skills and familiarity with electronic health records systems is preferred.
