Advanced Patient Care Coordinator

Job Summary
We are seeking an Advanced Patient Care Coordinator to join our team at Corewell Health.
About the Role
This is a full-time position working Monday through Friday with occasional on-call weekends. The successful candidate will be responsible for coordinating patient care, communicating effectively with staff and families, and providing leadership in a collaborative environment.
Educational Requirements
An Associate's degree in Nursing is required; a Bachelor's degree in Nursing is preferred. The ideal candidate will have at least 2 years of experience in OB-postpartum and breastfeeding support, or 1 year of experience in acute care, emergency, or rehab facility settings.
Certifications
A Basic Life Support (BLS) certification from the American Heart Association or American Red Cross is required. A Board Certified Lactation Consultant (BC-IBCLC) certification is also preferred.
Work Environment
The selected candidate will work in a fast-paced healthcare setting, prioritizing patient care while maintaining effective communication with colleagues and families. Physical requirements include standing, rotating trunk, and lifting up to 6 inches.
Salary Information
The estimated annual salary for this role is between $75,000 to $90,000, depending on experience and qualifications.
