About the Role

Mass General Brigham is seeking a skilled Nurse Care Coordinator Specialist to join our team in Belmont, Massachusetts.

The ideal candidate will have a strong background in nursing and excellent communication skills, with the ability to work independently and as part of a multidisciplinary team. The successful candidate will be responsible for coordinating post-acute care services for patients admitted to Medical, Surgical, Pediatric, or Obstetrical Services, as well as overseeing the coordination of care for high-risk patients.

Responsibilities
  • Develop and implement discharge plans for patients requiring post-acute care services or follow-up.
  • Collaborate with patients, healthcare teams, insurers, and community organizations to ensure seamless transitions of care.
  • Use a collaborative process of assessment, planning, facilitation, and advocacy to provide high-quality care coordination services.
  • Routinely review patients' health records and communicate with healthcare teams to ensure continued appropriate levels of care.
Requirements
  • Bachelor's degree in Nursing (BSN) preferred.
  • Current licensure as a Registered Nurse in the Commonwealth of Massachusetts, with at least three years' clinical experience.
  • Evidence of continued education and professional development, including CCM or ACMA certification.
  • Prior experience in general acute care hospital experience, healthcare reimbursement, quality improvement, and enhanced models of healthcare delivery.

What We Offer
  • A competitive salary: $93,000 - $117,000 per year, depending on experience.
