About the Role: We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Medically Focused Care Coordinator. As a key member of our interdisciplinary care team, you will be responsible for providing exceptional patient care in various community settings.

Job Summary: In this role, you will provide quality clinical services to patients with complex medical needs. You will work closely with our healthcare providers to develop personalized care plans that meet the unique needs of each patient. Your expertise in assessment, teaching, treatment, and documentation will ensure that our patients receive the highest level of care possible.

Responsibilities:

  • Provide skilled nursing care to patients in various community settings, including home care and rehabilitation facilities
  • Assess patients' physical and emotional needs, developing individualized care plans to address these needs
  • Collaborate with healthcare providers to coordinate patient care, ensuring seamless transitions between care settings
  • Deliver high-quality patient education, promoting self-care and independence
  • Document patient progress, maintaining accurate and timely records

Requirements:

  • Bachelor's degree in Nursing or related field
  • Current Massachusetts RN license
  • Minimum one year of experience in acute care setting; prior home care experience preferred
  • Ability to travel within designated geographic area
  • Strong communication and interpersonal skills, with ability to work effectively with diverse populations
