Social Work Specialist

Kaiser Permanente is seeking a skilled social worker to provide clinical services in a primary care or home health setting.
Key Responsibilities
- Evaluate patients' psychosocial assets and deficits to enable them to maintain independence.
- Provide short-term counseling, teaching, and brokering with other resources for caregiving needs.
- Consult and coordinate with multidisciplinary teams to establish a plan of care.
- Assess patient and family needs for care and communicate with other healthcare providers.
Requirements
- Master's degree in social work (MSW).
- Minimum one year of previous social work experience.
- Licensed Marriage and Family Therapist (Washington) or Licensed Advanced Social Worker License (Washington) or other relevant licenses.
