Job Summary:

Kaiser Permanente is seeking a highly skilled Social Work Specialist to provide clinical social work services in a primary care or home health setting. The ideal candidate will have a Master of Social Work degree and a minimum of one year of previous social work experience.

Key Responsibilities:
  • Provide psychosocial assessments and develop plans of care to enable patients to maintain the greatest level of independence.
  • Consult with multidisciplinary teams to coordinate care and provide support to patients and their families.
  • Evaluate patient and family needs through timely assessments, communication, and observation.
  • Modify plans of care based on patient and family response.
Requirements:
  • Master of Social Work degree.
  • Minimum one year of previous social work experience.
  • Excellent written and verbal communication skills.
  • Demonstrated success as part of a multi-disciplinary team and commitment to diversity.
Preferred Qualifications:
  • Two years of post-masters experience.
  • Care coordination experience in managed care systems, in-/outpatient settings, skilled nursing, or long-term care facilities.
  • Knowledge of social case management, conflict resolution, and team building practices.
  • Familiarity with Microsoft Office and electronic charting systems.
